Moving /Home folder to a /Home partition

I have an SSD disk of about 240Gb. On this disk is my EFI partition of 500Mb (sda1), my swap partition of 16Gb (sda2) and my / root partition of 150Gb (sda3) which holds my Home folder.
I also have an empty 1000Gb hard drive (sdb). I have read that it is good to have a large separate partition for Home and I'd like to change my Home folder to a 500Gb partition on sdb.
Is this possible to do without doing a reinstall?

Aravision thank you! That worked great and was very informative on peripheral things (how to use fdisk, gedit etc.) as well. My home partition is now separate from my system files and I did not have to reinstall.

@christianprcollins Can I ask that you do not create duplicate threads on the same subject. e.g. Change my Home folder to new partition
It just duplicates support effort and causes confusion.
