How do I get it back? Subscribe: all updates
It always says “custom” for me, and I can't set it to “all” anymore. 
Once you customize it, it will remain as showing as Custom.
But if you have settings set to "All", it will show as your settings indicate, given that "all" is your Custom Configuration.

Hmm, am I understanding this correctly?
I click “All updates” and then “Custom” is still displayed?
Yes.
Once you change a setting - it labels that as "Custom". I dislike that it operates that way, too.
